No Results with IPL? Why Your Patient Did Not See Results with Intense Pulsed Light or Broadband Light (IPL or BBL)

There is a big learning curve with IPL/BBL and is the most understood.


IPL is my absolute favorite technology to perform and teach on. If you haven't mastered it yet or are struggling with results, this blog post is for you.


IPL is the most difficult technology to learn, but can be the most rewarding once you get the hang of it.

Ive been using IPL for the last 14 years and have used the following devices in clinic:

Cynosure Starlux (formerly Palomar)

Inmode Lumecca

Rohrer Spectrum IPL

Sciton BBL

Alma Harmony IPL

Luvo Lucent IPL

Lumenis M22 IPL

Aesmed (VMS) IPL





Why Your Patient Did Not Get Results with Intense Pulsed Light (IPL or BBL)


Intense Pulsed Light (IPL), also known as Broadband Light (BBL), is one of the most effective treatments for pigmentation, redness, vascular lesions, and photodamage when used correctly. However, many providers become frustrated when their patients fail to achieve the expected results.

In most cases, the issue is not the technology itself—it's patient selection, treatment settings, or understanding the target being treated.

Here are the most common reasons your patient did not get results with IPL.


1. Your Fluence Was Too Low or You Didn't Perform Enough Passes

One of the most common mistakes providers make is staying too conservative with treatment settings.

If the energy delivered is too low, the target chromophore will not absorb enough heat to create the desired clinical effect. While treatment parameters provide a starting point, experienced providers know that clinical endpoints are often more important than simply staying within recommended settings.


A common mistake I see is that the IPL operator did not go higher on fluence because the recommended parameters stated not to go higher than a certain number. Or they were told by the clinical trainer to stay within a certain number.


Clinical Endpoints to Look For

Pigmented Lesions

  • Pigment darkens immediately after treatment

  • A mild red halo may appear around the lesion

Diffuse Redness

  • Increased erythema immediately following treatment

Vascular Lesions

  • Vessel blanching

  • Vessel darkening or turning purple

Without appropriate endpoints, the treatment may be too conservative to produce visible results.


Clinical endpoint for brown spots/redness. Brown spots should look darker with red ring around them. Redness will look reddened and will dissipate over 24 hours (won't see results for redness right away)

2. The Patient Had the Wrong Type of Pigmentation

Not all pigment responds equally to IPL.

One of the most important skills an aesthetic provider can develop is recognizing different types of pigmentation and understanding which lesions are appropriate for IPL treatment.

Conditions that often respond poorly to IPL include:

  • Melasma

  • Post-inflammatory hyperpigmentation (PIH)

  • Seborrheic keratoses

In some cases, melasma may actually worsen after treatment.

Lighter, deeper spots usually need a deeper penetrating laser such as a 1064/532 Nd:YAG.

Before reaching for the IPL handpiece, spend time accurately identifying the pigment you are treating. Proper diagnosis leads to better treatment selection and better patient outcomes.

3. Your Pulse Width Was Too Fast or Too Slow

Pulse width (pulse duration) determines how long energy is delivered to the target.

Selecting the appropriate pulse width is critical because different targets have different thermal relaxation times.

General Rule:

Smaller, more superficial targets

  • Require shorter pulse durations (faster)

Larger, deeper targets

  • Require longer pulse durations (slower)

Using an inappropriate pulse width may result in insufficient heating of the target or unnecessary heating of surrounding tissue, leading to poor clinical outcomes.

Note: Darker fitzpatricks require longer pulse durations


4. Your Wavelength or Filter Selection Was Incorrect

Many IPL platforms allow providers to change filters, altering which wavelengths are delivered to the skin.

Choosing the wrong filter can significantly reduce treatment efficacy.

A common example is treating superficial pigmentation with a deeper filter.

For example:

  • A 515 nm filter may be more effective for superficial pigmentation

  • A 560 nm filter may penetrate deeper and be less effective for certain superficial lesions

Understanding how wavelength selection influences depth of penetration is essential for successful IPL treatments.

IPL Tip: If possible, use multiple wavelengths and filters with multiple passes


5. The Patient Was Not an Ideal Candidate

Patient selection remains one of the most important factors in treatment success.

Patients with:

  • Darker Fitzpatrick skin types (avoid on fitz 5-6 despite what the company may tell you, no clinical studies have been done on fitz 5 and IPL, the melanin content is too concentrated)

  • Recent sun exposure

  • Active tanning

Often require lower treatment settings to minimize the risk of burns and post-inflammatory hyperpigmentation.

As treatment settings become more conservative, clinical efficacy may decrease.

The ideal IPL candidate typically has:

  • Lighter skin

  • Significant contrast between the target pigment and surrounding skin

  • Minimal recent sun exposure (avoid on sun worshippers or those that work outdoors)

The greater the contrast between the chromophore and the surrounding skin, the easier it is to selectively target the lesion.


Final Thoughts

Successful IPL treatments require much more than simply selecting a preset on the device. Providers must understand:

  • Chromophores and selective photothermolysis

  • Clinical endpoints

  • Pulse width selection

  • Filter and wavelength choices

  • Proper patient selection

  • Different presentations of pigment and vascular lesions

When these factors are understood and applied correctly, IPL can be one of the most rewarding and versatile treatments in aesthetic medicine.
