Model Inquiry

Please fill out the following form if you are interested in volunteering to be a model for one of my training courses located in Santa Clarita, California at Infinity Medspa.

All services are free of charge, however touch ups and additional units are not included

Requirements for models (PLEASE READ BEFORE SUBMITTING!)

-Must be ok with receiving injections by student under supervision

-MUST BE AN EXISTING PATIENT OF INFINITY MEDSPA within the last 5 years and have been treated by Marisa Amechi RN

-Filler and Botox naive will take priority or No filler in the same area within 2 years and No botox in same area the last 6 months.

-No pregnancy, breastfeeding

-No medical conditions or autoimmune disorders

-No history of consistent ibuprofen or aspirin use

-No alcohol use one week prior

-No anxiety or low pain tolerance

-No dental work/cleanings two weeks prior or two weeks after

-No history of vasovagal response (fainting or passing out with needles)

-No history of herpes simplex or cold sores

-All trainings are filmed and recorded-OK with social media and marketing use (Photos and videos can be used on social media, website, publications, flyers and commercials)

-Must be ok with possibility of bruising, swelling for up to 2 weeks

-Must not be on tight schedules in case training runs behind or runs ahead of schedule

-Must be prompt and on time

I agree to be injected by an injection student under supervision of Marisa Amechi RN. All students are licensed.

I agree I do not have any medical conditions including autoimmune disorders, pregnancy and breastfeeding.

I agree to be filmed and photos/videos may be used for marketing, websites, social media, flyers or commercials.

I agree I do not have a fear of needles, history of lightheadedness or passing out with needles and have a high pain tolerance

I agree to avoid taking ibuprofen, aspirin, fish oil and alcohol one week prior to reduce bleeding or bruising

I agree that any touch ups for Botox or Fillers are not included. All Botox units and fillers during the training are free

I agree there is a possibility of bruising/swelling and takes two weeks to see full results

I agree if I cannot attend the training to contact Marisa two weeks prior so I can refill your spot. Any last minute cancellations or no shows will revoke any oppurtunity to be a model in the future

I agree there is a screening process and by submitting this form does NOT guarantee selection. All models will be contacted if selected
